What the .health registry actually polices
The word health in a web address makes a promise, and the fight when this extension was applied for was about who would hold anyone to it. What ICANN and DotHealth, LLC signed on 11 February 2015 is a set of public interest commitments, and they describe a registry that watches what sites do rather than one that checks who registers.
Under those commitments the registry undertakes to prohibit the illicit promotion or sale of prescription drugs, controlled substances, tainted dietary supplements and ingredients for psychoactive highs; to monitor the extension proactively, itself or through contractors; to run rapid takedown measures including cooperation with international law enforcement; and to prohibit abusive or malicious commercial practices aimed at consumers and children. It reserves the right to lock, cancel, transfer or take ownership of any domain, temporarily or permanently, where a site appears to put a registrant or user at risk. Registrants holding sensitive health data are required to put appropriate security in place.
What none of it includes is a check on you. There is no practitioner register, no licence number to upload and no professional body to satisfy. .health is enforced after the fact rather than gated at the door, and buying one confers no credential on anybody.
Who is allowed to register .health?

- No credential check
- No medical qualification is verified, no licence number is requested and no professional body signs anything off. A .health address proves nothing about the person behind it, so do not present it to patients as though it did.
- What the registry commits to police
- The public interest commitments name the illicit promotion or sale of prescription drugs, controlled substances, tainted dietary supplements and ingredients for psychoactive highs, plus abusive or malicious commercial practices targeted at consumers and children.
- Proactive monitoring
- The registry commits to monitoring the extension itself or through third-party contractors, to acting against rogue internet pharmacies operating in it, and to registry-specific rapid takedown measures including cooperation with international law enforcement agencies.
- A security duty lands on you
- Registrars must require registrants who collect and maintain sensitive health and financial data to implement reasonable and appropriate security measures as defined by applicable law. On a site taking symptoms or appointment details that is a working obligation, not boilerplate.
- If the registry decides against you
- It reserves the right to lock, cancel, transfer or take ownership of a domain name, temporarily or permanently, where the name appears to threaten the stability, integrity or security of the extension or its registrar partners, or to put the safety of any registrant or user at risk. That is a wider power than most extensions reserve.

For a practice with a waiting room, the .co.uk wins on the things that fill appointments: it is a country signal to Google, it is what patients type without thinking, and it costs a fraction of a .health every year. Nothing about .health improves local visibility, because Google reads it as a generic extension.
Where .health makes sense is national or international publishing, where the subject matters more than the postcode, and defensive registration around a clinical brand. If you are weighing them for a single UK clinic, register the .co.uk as the working address and treat the .health as an optional extra rather than an upgrade.
