What a .care domain does not prove
Nobody checks. That is the first thing a care provider should understand about .care, because the extension gets sold as though it were a credential. Binky Moon runs it on ICANN's standard base agreement, unsponsored, with no community specification, no eligibility rules and no verification step anywhere in the process. A care home group, a nursery, a charity and somebody with a laptop can all register one this afternoon.
What the registry does have is an acceptable use policy, and one line in it lands squarely here. Identity Digital lists holding yourself out as having a status, credential or authorisation associated with a TLD when it does not exist as abusive use, next to phishing and malware, and reserves the right to suspend, cancel or transfer a name over it. The extension is not a licence, and claiming it is one is the sort of thing you can lose the domain for.
Read that way, .care earns its place as a naming tool rather than a trust badge. It works when the word finishes your name cleanly: a home care service whose brand is a single word, a nursery whose .co.uk went a decade ago, a sector directory, a jobs board for care staff. It works less well as the main address for a UK provider, because families search for a care home near them and the .co.uk carries the UK signal that a word extension does not. And it says nothing about your standing. In England that is what the Care Quality Commission register is for, and no domain ending substitutes for an entry on it.

.care vs .co.uk: which should you register?
For a UK care provider this is not a close contest and it is worth saying plainly. A family looking for a care home in Kettering searches for a care home in Kettering. Google reads .co.uk as a UK site; it reads .care as a word with no geography attached. On local search alone, the .co.uk wins.
Where .care changes the picture is naming. If your organisation is called something whose .co.uk went in 2004, the choice is between an awkward address with a hyphen or a filler word in it and a clean one that reads properly. The clean one is easier to say down the phone and easier to print on a brochure that will be posted to a relative in another county.
The usual answer is both: the .co.uk as the address you advertise and the .care as the memorable one, redirecting to the same site. Nominet privacy on the .co.uk also keeps a sole trader's home address out of the public record, which the .care cannot do for free.

The .care is worth adding when your name reads better with it, so use it as a second, memorable address pointing at the same site rather than replacing the one that earns the local visibility.
Some will not recognise it, and recognition is what trust is made of at first glance. Families researching care are often doing it in a hurry and under stress, and an unfamiliar ending is one more thing to think about. What actually builds trust is the regulator listing, real photographs, named staff, plain fee information and a phone number that gets answered. The domain is a label on that work, not a substitute for it.
