Nobody verifies your credentials on a .doctor domain
Almost every buyer arrives assuming .doctor is verified, that somebody checks a medical register before the name is issued. Nobody does. The registry agreement Binky Moon signed with ICANN on 2 June 2016 carries an explicit public interest commitment that second level registrations will be open and available to lawful registrants, that the registry will not limit eligibility based on identity, and that it will not shut out lawful users associated with a minority usage of the word. In the UK the check happens somewhere else entirely, and it has teeth. Section 49 of the Medical Act 1983 makes it an offence to wilfully and falsely take or use the title of physician, doctor of medicine or general practitioner, or any description implying registration under that Act, punishable by a fine on summary conviction. A domain does not make the claim by itself; the words on the site do. If you are on the GMC register a .doctor is accurate. If you are not, the extension is an invitation to a question you would rather not answer.
Then there is the price, which is the practical objection. At the figure on this page .doctor is the most expensive extension in this group, more than nine times a .co.uk and above .health, .clinic and .dental. For a practice whose patients search from the next street, that money buys nothing a .co.uk would not.

.doctor vs .clinic: which should you register?
Both are Binky Moon extensions under Identity Digital, both open to anyone, and both delegated in the same wave: .clinic in April 2014, .doctor in July 2016. The useful distinction is person against place.
.doctor names an individual. It reads as a practitioner, which suits a consultant, a private specialist or anyone whose own name is the practice. .clinic names a premises. It reads as a building with a reception desk and several people in it, which suits a partnership, a dental practice or a physiotherapy centre far better, and it costs a good deal less each year.
Neither carries a credential. Neither is checked. The difference that will actually reach a patient is which of the two describes what they will walk into, and for most UK practices that is the clinic rather than the doctor.
